Custom Product Impression Information and Order Form.
If you are purchasing custom products here online please take a copy of the Custom Product Order Form with you to your audiologist appointment. Be sure to fill in the name of the person the impressions are for and the Audiologists' information. In the "Bill To, Ship To and the Items Requested" fields you can simply write the Online Order Number you received from the store when you checked out. Include the filled out form when you send in your ear impressions. After placing your online order please send ear impressions in a timely manner to avoid delay in service to you.
